Health Minister Discretionary Grant Financial assistance of Rs 1,00,000/-

राष्ट्रीय आरोग्य निधि (आर.ए.एन.) के तहत आने वाली जानलेवा बीमारियों के लिए जहां मुफ्त चिकित्सा सुविधाएं नहीं हैं ऐसे सरकारी अस्पतालों में भर्ती/उपचार पर होने वाले खर्च के एक हिस्से को चुकाने के लिए स्वास्थ्य और परिवार कल्याण मंत्रालय द्वारा एक स्वास्थ्य योजना। केवल वे लोग जिनकी वार्षिक पारिवारिक आय 1.25,000 रुपये और उससे कम है, वे स्वास्थ्य मंत्री के विवेकाधीन अनुदान (एच.एम.डी.जी.) से वित्तीय सहायता के लिए पात्र हैं। इस योजना के तहत सुपर स्पेशियलिटी सरकारी अस्पतालों/संस्थानों सहित सरकारी अस्पतालों में इलाज के लिए वित्तीय सहायता प्रदान की जाती है

फ़ायदे

सुपर स्पेशियलिटी सरकारी अस्पतालों/संस्थानों सहित सरकारी अस्पतालों में इलाज के लिए वित्तीय सहायता।
  1. यदि इलाज की अनुमानित लागत 1,00,000/रुपये तक है तो 50,000 रुपये की वित्तीय सहायता .
  2. यदि इलाज की अनुमानित लागत 1,00,000/- रुपये से ऊपर और 1,50,000/- रुपये तक है तो 75,000 रुपये की वित्तीय सहायता।.
  3. यदि उपचार की अनुमानित लागत 1,50,000/- रुपये से अधिक है तो 1,00,000/- रुपये की वित्तीय सहायता।.
 

पात्रता

  1. आवेदक को केंद्र या राज्य सरकार का कर्मचारी नहीं होना चाहिए।
  2. आवेदक को सरकारी अस्पताल (सुपर स्पेशियलिटी सरकारी अस्पतालों/संस्थानों सहित) में अस्पताल में भर्ती/उपचार किया जाना चाहिए।
  3. आवेदक की कुल वार्षिक पारिवारिक आय प्रति वर्ष 1,00,000 रुपये से अधिक नहीं होनी चाहिए.

अपवाद

निम्नलिखित मामले में आवेदक एच.एम.डी.जी. के तहत लाभ प्राप्त करने के पात्र नहीं हैं::
  1. आवर्ती व्यय को शामिल करते हुए लंबे समय तक उपचार की अनुमति नहीं है।
  2. सामान्य प्रकृति के रोग जिनका इलाज महंगा नहीं है.
  3. टी.बी. के मामले
  4. निजी अस्पताल में इलाज होने के मामले में
  5. केंद्र और राज्य सरकार के कर्मचारी।.

आवश्यक दस्तावेज़

    1. निर्धारित प्रपत्र में आवेदन पत्र उपचार करने वाले चिकित्सक द्वारा विधिवत हस्ताक्षरित और सरकारी अस्पताल/ संस्थान के चिकित्सा अधीक्षक द्वारा प्रतिहस्ताक्षरित।
    2. रोगी को दी जाने वाली वित्तीय सहायता की मात्रा है- (क) 75,000/- रुपये, यदि उपचार की अनुमानित लागत 1,25,000/- रुपये तक है( ख) 1,00,000/- रुपये यदि उपचार की अनुमानित लागत 1,25,000 रुपये से अधिक है और 1,75,000/- रुपये तक है और (ग) 1,25,000/- रुपये से अधिक है, तो उपचार की अनुमानित लागत 1,25,000/- रुपये है।
    3. बीडीओ/तहसीलदार/कलेक्टर/एसडीएम (प्रमाण पत्र जारी करने वाले प्राधिकारी का पूरा नाम, पदनाम, टेलीफोन नंबर और पूर्ण आधिकारिक पता उद्धृत करते हुए) से मूल रूप से आवेदक और उसके परिवार के सदस्यों के सभी स्रोतों से व्यवसाय और मासिक / वार्षिक आय के बारे में एक प्रमाण पत्र।
    4. राशन कार्ड की प्रति।

Funding pattern of Scheme Budgetary provisions are made to fund the HMDG Scheme.
Beneficiaries & eligibility criteria Only those having annual family income up to Rs.1.25,000/- and beloware eligible for financial assistance from Health Minister’s Discretionary Grant (HMDG) to defray a part of the expenditure on Hospitalization/treatment in Government Hospitals, in cases where free medical facilities are not available.
Type of benefits Financial assistance towards treatment in Government hospitals including super specialty Government hospitals / institutes, as per guidelines.
How to avail benefits To avail financial assistance under HMDG, the patient is required to submit :

  1. Application form in prescribed proforma duly signed by the treating doctor and countersigned by the Medical Superintendent of the Government hospital/institute.
  2. Copy of the income certificate.
  3. Copy of the ration card.
  4. Quantum of financial assistance granted to the patient is- (a) Rs.75,000/-, if the estimated cost of treatment is up to Rs.1,25,000/-(b) Rs.1,00,000/- if the estimated cost of treatment is above Rs.1,25,000/- and up to Rs.1,75,000/- and (c) Rs.1,25,000/- if the estimated cost of treatment is above Rs.1,75,000/-.

Details

A health scheme by Ministry of Health & Family Welfare for financially poor patients to defray a part of the expenditure on Hospitalization/treatment in Government Hospitals,for life threatening diseases covered under Rashtriya Arogya Nidhi (RAN), in cases where free medical facilities are not available. Only those having annual family income up to Rs.1.25,000/- and below are eligible for financial assistance from Health Minister’s Discretionary Grant (HMDG). Under this scheme, Financial assistance is provided towards treatment in Government hospitals including super specialty Government hospitals / institutes.

Health Minister Discretionary Grant Benefits

Financial assistance towards treatment in Government hospitals including super specialty Government hospitals / institutes.
  1. Financial Assistance of ₹50,000/- if the estimated cost of treatment is up to ₹1,00,000/- .
  2. Financial Assistance of ₹75,000/- if the estimated cost of treatment is above ₹1,00,000/- & up to ₹1,50,000/-.
  3. Financial Assistance of ₹1,00,000/- if the estimated cost of treatment is above ₹1,50,000/-.
 

Health Minister Discretionary Grant Eligibility

  1. The applicant must not be an employee of either Central or State Govt.
  2. The applicant must have been Hospitalized / Undergoing Treatment in a Government Hospital (including super specialty Government hospitals / institutes).
  3. The Total Annual Family Income of the applicant must not exceed ₹1,00,000/- per annum.

Exclusions

The following cases are not eligible to avail benefits under HMDG:
  1. Prolonged treatment involving recurring expenditure is not permissible.
  2. diseases of common nature where treatment is not expensive.
  3. T.B. cases
  4. Cases for treatment in private hospital.
  5. Central as well as State Government employees.

Health Minister Discretionary Grant Documents Required

    1. Application form in prescribed proforma duly signed by the treating doctor and countersigned by the Medical Superintendent of the Government hospital/institute.
    2. Quantum of financial assistance granted to the patient is- (a) Rs.75,000/-, if the estimated cost of treatment is up to Rs.1,25,000/-(b) Rs.1,00,000/- if the estimated cost of treatment is above Rs.1,25,000/- and up to Rs.1,75,000/- and (c) Rs.1,25,000/- if the estimated cost of treatment is above Rs.1,75,000/-.
    3. A Certificate in original from the BDO/ Tehsildar/Collector /SDM (quoting full name, designation, telephone number and complete official address of the certificate issuing authority) regarding occupation and monthly/yearly income of the applicant and his/her family members from all sources.
    4. Copy of the ration card.
